Social media’s love of sharp cheekbones and hollowed cheeks has made buccal fat removal one of the most requested procedures of recent years. Yet it is an irreversible operation, and without disciplined patient selection, today’s chiselled face can become tomorrow’s prematurely aged one. What Buccal Fat Is — and What Happens When It Goes

The buccal fat pad is an encapsulated fat compartment deep in the cheek, between the chewing muscles. Removing part of it through a small incision inside the mouth slims the lower-mid cheek and deepens the shadow beneath the cheekbone.

The critical property: this fat differs from superficial fat — it does not shrink or grow with weight changes, and once removed it does not return. Both the power and the risk of the procedure live in that permanence.

Who Gets a Good Result

The ideal candidate profile is narrow: a round, full face; clearly prominent buccal fullness; good bone structure with adequate cheekbone projection; and usually age 25–40. In this profile, conservative (partial) removal produces an elegant refinement.

In a lean or normally full face, the procedure delivers a subtle effect today — and from the forties onward, a real risk of a hollow, tired midface, because facial fat naturally declines with age and the buccal reserve is one of the tissues that buffers that loss.

The 20-Year Perspective

A core principle of aesthetic surgery: do not mortgage tomorrow’s face for today’s trend. Aggressive buccal removal in youth is a leading cause of the ‘over-carved’ midface of middle age — and the correction (fat grafting) never fully restores original anatomy.

Procedure and Recovery

Performed through the mouth under local anaesthesia or sedation, in 30–45 minutes, with no external scar. Expect notable cheek swelling in the first days — paradoxically, the final slimming look takes 3–6 months, so never judge the result by week one.

Oral hygiene protocol (rinses, soft diet) matters in the first week; social downtime is 3–5 days.
